Panchakarma literally means five actions. These are not interchangeable wellness treatments  each one targets a specific dosha imbalance and works through a specific physiological pathway. In 25 years of clinical practice, I have never administered the same combination of therapies to two different patients.

Kapha dosha

Vamana

Therapeutic emesis

A controlled, medically supervised elimination therapy designed to expel excess Kapha from the upper respiratory and digestive tract. Never done without a proper preparation phase  the body must be properly oleated and fomented before this therapy is safe and effective.

Virechana

Therapeutic purgation

Addresses excess Pitta through controlled purgation using specific herbal formulations. Directly improves liver metabolic function and the body’s ability to process hormones correctly. In my experience, Virechana produces some of the most dramatic and lasting results of any single Panchakarma procedure when properly indicated.

The most important of the five therapies per classical Ayurvedic texts and after 25 years of clinical practice, I agree with that assessment. It directly addresses Vata  the most common root cause of chronic conditions I see in practice. Available in two forms: Anuvasana Basti (oil-based) and Niruha Basti (decoction-based).

Panchakarma, in contrast, is a personalized program built around the individual’s Prakriti, current imbalances, Agni (digestive strength), and complete health history all designed under clinical supervision. The specific therapies used, their sequence and duration, and the preparatory and post-treatment protocols are all individualized. Two patients presenting with identical symptoms can require completely different Panchakarma programs

Certain therapies may need to be avoided or adjusted depending on the specific medication and condition.
