In the last five years I have seen a significant increase in patients presenting with hair fall after keratin treatment. They come in confused they did the keratin treatment to improve their hair, and instead they are losing more of it than before.

Here is the honest clinical answer: yes keratin treatment can cause hair fall. But the mechanism is more specific than most patients understand. And the solution is not simply stopping keratin treatments.

WHAT IS KERATIN TREATMENT?

Keratin treatment is a chemical hair smoothing procedure that coats the hair shaft with a keratin protein formula typically combined with formaldehyde or formaldehyde releasing chemicals and then seals it with high heat from a flat iron at 230°C or higher.The procedure straightens, smooths, and adds shine to the hair. Yes keratin treatment can cause hair fall through three distinct mechanisms:

Mechanism 1 Formaldehyde scalp damage:
Most keratin treatments contain formaldehyde or formaldehyde-releasing compounds methylene glycol, formalin, methanediol. At the high temperatures used during the treatment, these compounds release formaldehyde gas directly onto the scalp. Formaldehyde is a known irritant that damages the scalp tissue, disrupts the sebaceous glands, and creates a toxic microenvironment around the hair follicles.

Mechanism 2 Heat damage to the follicle:
The flat iron temperatures used to seal keratin treatment 200-230°C are significantly higher than the temperature the scalp tissue can safely tolerate. Repeated application of this heat directly over the scalp creates thermal damage to the follicle opening and the arrector pili muscles that control follicle positioning. Over time this thermal damage weakens the follicle anchor and hair fall follows.

Mechanism 3 Pitta aggravation in Ayurvedic terms:
In classical Ayurveda, the scalp is governed by Bhrajaka Pitta the subdosha of Pitta that manages heat metabolism in the skin and scalp. Keratin treatment directly aggravates Bhrajaka Pitta through chemical heat and thermal heat simultaneously. This aggravated Pitta then attacks the Asthi Dhatu the bone and hair tissue producing the characteristic diffuse thinning and increased shedding seen after keratin treatment.

DOES HAIR FALL AFTER KERATIN TREATMENT STOP ON ITS OWN?

Sometimes and sometimes not. Here is what determines the outcome:

Hair fall after keratin treatment stops on its own when:

  • The treatment was done once or twice with minimal scalp contact
  • The patient has strong Asthi Dhatu and good baseline hair health
  • No underlying hormonal or nutritional deficiency exists
  • The patient stops keratin treatment and gives the scalp 3-6 months to recover

Hair fall after keratin treatment does not stop on its own when:

  • The treatment has been done repeatedly over several years
  • Formaldehyde damage has created chronic scalp inflammation
  • An underlying Pitta imbalance or hormonal condition was already present before the treatment
  • The patient continues keratin treatment while experiencing hair fall

In my clinical experience patients who come to me with post-keratin hair fall typically have an underlying Pitta imbalance that the keratin treatment has triggered or worsened. Stopping the treatment is necessary but not sufficient. The Pitta imbalance and follicular damage need active clinical treatment.

HOW AYURVEDA TREATS HAIR FALL AFTER KERATIN TREATMENT

The Ayurvedic approach to post-keratin hair fall addresses three things simultaneously:

Step 1 Clear the accumulated Pitta from the blood and scalp:
