Polycystic Ovary Syndrome presents differently in every patient. Some women have irregular cycles. Some have no periods for months. Some have heavy, painful bleeding. Some have acne, weight gain, and hair fall. Some have all of the above. This variability is not a sign that PCOS is complicated it is a sign that the root cause is different for each person.

In classical Ayurveda, PCOS or PCOD is understood through three dominant patterns. Identifying which pattern is dominant  and in most patients it is a combination  is the foundation of every PCOS treatment I design at AClinic.

Kapha dominant most common

Weight gain, sluggish metabolism, irregular cycles

Presents as weight gain concentrated around the abdomen and hips, insulin resistance, oily skin, and cycles delayed by weeks or months with heavy, clotted flow. Kapha accumulation in the channels of reproduction directly suppresses ovulation by blocking hormonal signaling pathways.

Vata dominant

Scanty or absent periods, anxiety, hair fall

Less common but often more distressing. Scanty or absent periods, severe anxiety, sleep disruption, hair fall, and dry skin. These patients are often underweight or normal weight with PCOS — which confuses conventional diagnosis. Root cause: Vata aggravation disrupting the rhythmic hormonal cycle.

Pitta dominant

Acne, hair loss, excess facial hair, heavy periods

Inflammatory presentation  acne, scalp hair loss, excess facial hair, PMS with significant anger and irritability, and periods that are heavy with dark blood and significant pain. Liver function is often compromised in this pattern, driving androgen excess.

Root Causes Behind Persistent PCOS & PCOD

PCOS does not arise from a single cause. In my clinical experience, the following factors are almost always present in some combination:

Insulin resistance & metabolic imbalance

Present in the majority of PCOS patients regardless of body weight. Ayurveda addresses this through specific dietary protocols, herbal formulations that improve cellular glucose uptake, and Panchakarma therapies that restore metabolic function.

Chronic stress & cortisol dysregulation

Cortisol elevation from sustained psychological stress directly suppresses the hypothalamic-pituitary-ovarian axis — the hormonal cascade that governs the menstrual cycle. One of the most underdiagnosed drivers of PCOS in urban women aged 25-38.

Gut health & estrogen recirculation

The gut microbiome directly influences estrogen metabolism. When gut function is impaired, estrogen recirculates rather than being eliminated  contributing to the estrogen-progesterone imbalance that drives PCOS symptoms

Coexists with PCOS in a significant percentage of patients. I always assess thyroid function as part of my initial evaluation — treating PCOS without addressing an underlying thyroid imbalance produces incomplete results.

Udvartana

For Kapha-dominant PCOS with significant weight gain and insulin resistance. Dry herbal powder massage directly stimulates lymphatic circulation, breaks down subcutaneous fat deposits, and improves cellular metabolism. Combined with specific dietary protocols targeting insulin sensitivity.

Therapy 04 — For stress-driven PCOS

Shirodhara

For Vata-dominant and stress-driven PCOS presentations. Directly calms the hypothalamic stress response, reduces cortisol, and supports the restoration of regular hormonal rhythms. Patients consistently report significant improvements in sleep, anxiety, and emotional regulation within the first week of treatment.

Every PCOS consultation begins with a complete Ayurvedic assessment  Prakriti evaluation, Vikriti analysis, Nadi Pariksha (pulse diagnosis), menstrual history, dietary history, stress load, sleep patterns, and any existing investigations including ultrasound, hormonal panel, and thyroid function.

I do not prescribe the same herbal formula or the same diet to every PCOS patient. Two women with identical ultrasound findings and identical hormonal panels may need completely different treatment protocols based on their underlying dosha pattern and digestive strength.

Most patients I treat for PCOS have already tried at least two or three conventional approaches with incomplete results. If that is your situation   you are not out of options. You simply have not yet addressed the root cause. That is what I am here to find.

Can Ayurveda cure PCOS permanently?

Ayurveda does not use the word cure  it uses the word balance. When the root doshic imbalance driving your PCOS is corrected through treatment, dietary change, and lifestyle adjustment, the symptoms resolve and the cycle regulates. In clinical experience, patients who complete a full Panchakarma program and follow the prescribed protocol see lasting improvement  not temporary suppression.

Most patients see initial improvement in cycle regularity within 2-3 months of consistent treatment. Significant hormonal and metabolic correction typically takes 4-6 months of a complete protocol including Panchakarma, herbal formulations, and dietary adjustment. Patients with longstanding PCOS 5 years or more  may require 8-12 months for full correction.

Yes  in most cases. Dr. Ajayita assesses each patient’s current medications during the initial consultation and designs the Ayurvedic protocol accordingly. Some Panchakarma therapies may need to be modified or sequenced around existing medications. A complete medication history review is mandatory before any treatment begins at AClinic.
