THE THREE STAGES OF PANCHAKARMA TREATMENT
Classical Panchakarma treatment has three distinct stages:
STAGE 1 Purvakarma (Preparatory procedures):
Before any of the five main therapies can be administered, the body must be prepared. Purvakarma loosens and mobilizes the accumulated Ama from the deep tissues to the digestive tract where it can be eliminated.
Purvakarma includes:
Snehana internal and external oleation using medicated ghee and oils
Swedana therapeutic heat treatment to open the channels and soften the tissues
Duration: 3-7 days depending on the patient’s constitution and the depth of accumulation.
STAGE 2 Pradhana Karma (Main Panchakarma procedures):
The five main therapeutic procedures Vamana, Virechana, Basti, Nasya, and Raktamokshana administered in the sequence determined by the assessment. Only the procedures indicated for your specific condition and dosha pattern are used.
Duration: 7-21 days depending on the protocol.
STAGE 3 Paschatkarma (Post treatment care):
After the main procedures, the body needs specific support to consolidate the therapeutic effect and restore digestive strength. Paschatkarma includes a specific dietary protocol Samsarjana Krama that gradually reintroduces foods in a sequence that matches the restored digestive capacity.

THE FIVE PANCHAKARMA THERAPIES EXPLAINED
VAMANA Therapeutic Emesis:
Controlled therapeutic vomiting induced through specific medicated preparations. Vamana directly eliminates excess Kapha from the stomach and lungs. Used for Kapha dominant conditions chronic respiratory disorders, obesity, skin diseases, and metabolic sluggishness. Vamana is the least commonly used of the five therapies in modern clinical practice and is only administered when the patient is strong enough to tolerate it.
VIRECHANA Therapeutic Purgation:
Medicated purgation that directly eliminates excess Pitta from the liver, small intestine, and blood. Virechana is the most commonly used Panchakarma therapy in my clinical practice. Used for PCOS hormonal imbalance, hair fall with early greying, skin conditions, liver disorders, digestive dysfunction, and chronic inflammatory conditions.
BASTI Medicated Enema Therapy:
Classical Ayurveda calls Basti the king of all Panchakarma therapies because Vata, the primary dosha responsible for all movement in the body, is governed through the colon. Medicated oils or herbal decoctions administered through the rectum directly nourish and cleanse Vata. Used for joint pain, neurological conditions, reproductive health, Vata-dominant hair fall and tissue depletion.
NASYA Nasal Administration:
Medicated oils administered through the nasal passages the direct pathway to the brain and nervous system. Nasya directly nourishes the scalp, the cranial nerves, and the hypothalamic pathways. Used for stress-driven hair fall, migraine, sinusitis, cervical spondylosis, and neurological conditions. RAKTAMOKSHANA Therapeutic Bloodletting:
The most rarely used of the five therapies in modern clinical practice.Used for specific blood borne conditions chronic skin disorders, localized joint inflammation, and certain types of pain. Performed only when clearly indicated after assessment.
